Nomination forms can be downloaded from the Forbes Sports Person of the Year Facebook page.
Nominations close January 31st January 2017 and all sporting clubs are urged to nominate someone for every category, recognising your club’s achievers for 2016.
All nominations are judged by an independent judging panel with winners to be announced at the Annual Awards Presentation Dinner on Saturday March 4 at the Forbes Services Memorial Club commencing at 6.30pm.
